3. PMMIC will request:
   1. Completed PMMIC Agency Agreement
   2. Personal Insurance Background Form
   3. W-9
   4. E&O Declarations Page
   5. Copies of Agent & Agency Licenses
4. Get approved and start offering your clients the most comprehensive pollution liability coverage in the industry!
